' [NGO Name] ’ provides you with a Cell Phone NOKIA C2-01 model , Nokia  charger, Vodaphone/Idea/Reliance Sim -card and Commcare Mobile application introduced by Dimagi innovations Pvt. Ltd,  to use for your work as a identified ASHA Worker of our target community …………………………………………..working as 'Community Health Volunteers ' (further CHV).

 

The NOKIA Cell phone comes with responsibilities; this contract will describe the ownership, usage and maintenance of the telephone.

 

1) Ownership:

  • The Cell phone  will be property of “NGO” based at said location.
  • All Cell phone have a unique identification number and a “ NGO " inscription and are registered by  “WORLD VISION INDIA, ADP MELGHAT".
  • All Cell phones will come with a unique Vodaphone   SIM-card. “ NGO " will register all SIM-card according to the Law of Telephone regulatory Authority of India( further TRAI).

 

2) Usage:

  • The Cell phone will be used as a job aid for “Project Name ” work under Nutrition Project for NGO "
  • The Cell phone will be used solely by the assigned ASHA Worker  (CHV)
  • The Cell phone will be used during all HBC visits following the instructions from the trainings and the CHV Cell phone guideline (attached)
  • The data will be sent after every visit in case of no/limited connectivity the data will be send as soon as the connectivity is back.
  • The Cell phone may be used for private use as long as it does not interfere with the HBC work. Private use credit will be reduced from the CHV's monthly incentives.
  • Use of Cell phone is strictly prohibited by the any other individual person except ASHA Worker(CHV).

 

3) Maintenance and credit

  • The CHV as a individual is responsible for the maintenance of the Cell phone.
  • The CHV will assure sufficient battery while using Cell phone for visiting the clients.(Community).
  • The CHV will assure there is sufficient credit for sending the data at any cost.
  • ' NGO " will provide the CHV with  Rs. 20/ - as monthly recharge credit for data sending.
  • NGO " will check the number of data sets send against the credit spend, the difference for private use will be reduced from the CHV's monthly incentives If found unsatisfactory usage.

 

4) Lost, stolen and/or broken

  • The Cell phone will be protected against stealing, losing and breaking by the CHVs.

 

  • In case the Cell phone breaks, “ NGO " will check the cause of the problem and solves the problem if possible. If the problem is not related to miss-use of the Cell phone, the cell phone will be replaced.

 

  • Where the Cell phone  is stolen, lost or broken due to negligence of the CHV Then the CHV SHALL be responsible for the replacement of the Cell phone. This will be done  by recovering either  full purchased “AMOUNT” or  deducting 'AMOUNT', from your monthly allowance till the total amount is recovered as per the norms.

 

This agreement has been explained to me in Marathi & Hindi and I Clearly understood  the terms and condition . I agreed with the  all the above mentioned  Cell phone rules and regulations.
